Original ceiling fan died and turns out there was never a vent run to the outside, so steam had been going into the attic insulation for years.  Cut the ceiling sheetrock, replaced insulation, drilled through joists to outside, installed vent and new fan.  Finished ceiling with patch, tape, mud, sand and primer.
